How the Service Works
- Local Coordination: GPDQ works with NHS and council teams to select sites and target populations.
- Clinic Setup: We mobilise pop-up or recurring clinics in community settings like shopping centres, places of worship, and libraries.
- Screening and Lifestyle Assessment: Checks include cholesterol, blood pressure, BMI, and blood sugar — plus questions on diet, alcohol, smoking, and activity levels.
- Personalised Advice and Signposting: Participants receive tailored guidance and are referred to local services or digital tools.
- Reporting and Follow-Up: Outcomes are anonymised, reported back to commissioners, and used to shape local strategy.
