Bringing Multidisciplinary Care Closer to Home
GPDQ’s CQC-registered Clinics are designed to support ICBs, Trusts, and community teams to deliver more care outside of hospitals. Whether run in person or virtually, these clinics provide targeted support for long-term conditions, acute presentations, and high-risk groups — relieving pressure on GP practices, A&E, and outpatient services.
Why Clinics Matter
-
Improve access to primary and specialist care
-
Reduce avoidable A&E and urgent care usage
-
Free up GP capacity for complex care
-
Support earlier diagnosis and treatment
-
Deliver care locally for greater convenience and equity
-
Reduce overall system costs
How the Service Works
- Local Engagement and Targeting: We collaborate with NHS partners to design clinic formats that address local priorities.
- Setup and Mobilisation: Clinics are delivered in surgery or offered virtually, based on local need and pathways.
- Clinical Delivery: Our multidisciplinary team sees booked or walk-in patients for condition-specific or general care.
- Onward Management and Signposting: Patients are supported with care plans, referrals, and connections to wider services.
- Data Capture and Feedback: All consultations are recorded and reported back via appropriate NHS systems.
